Global, regional, and national burden of disorders affecting the nervous system, 1990–2021: a systematic analysis for the Global Burden of Disease Study 2021

BackgroundDisorders affecting the nervous system are diverse and include neurodevelopmental disorders, late-life neurodegeneration, and newly emergent conditions, such as cognitive impairment following COVID-19. Previous publications from the Global Burden of Disease, Injuries, and Risk Factor Study estimated the burden of 15 neurological conditions in 2015 and 2016, but these analyses did not include neurodevelopmental disorders, as defined by the International Classification of Diseases (ICD)-11, or a subset of cases of congenital, neonatal, and infectious conditions that cause neurological damage. Here, we estimate nervous system health loss caused by 37 unique conditions and their associated risk factors globally, regionally, and nationally from 1990 to 2021.MethodsWe estimated mortality, prevalence, years lived with disability (YLDs), years of life lost (YLLs), and disability-adjusted life-years …

P016 Cardiovascular disease and rheumatoid arthritis among people with a history of cancer

Background/Aims Cardiovascular disease (CVD) is connected to many comorbidities and vascular diseases. Cancer also has certain inflammatory impact on the physiological mechanisms. However, the precise connection between these variables are unknown. In this research, we determined if there was an association between cardiovascular disease and overall mortality and how this connection may be modified by rheumatoid arthritis. Methods The National Health and Nutrition Examination Survey (NHANES) survey was conducted by the National Center for Health Statistics (NCHS) for the six cycles or years of 1999 to 2010 with mortality follow-up through December 31, 2019. This data is published biennially. Our study was conducted on participants with a history of cancer that were aged 40 years or older living in the United States with cardiovascular disease. CVD was …

Improved all-cause mortality with left bundle branch area pacing compared to biventricular pacing in cardiac resynchronization therapy: a meta-analysis

BackgroundLeft bundle branch area pacing (LBBAP) has emerged as a physiological alternative pacing strategy to biventricular pacing (BIVP) in cardiac resynchronization therapy (CRT). We aimed to assess the impact of LBBAP vs. BIVP on all-cause mortality and heart failure (HF)-related hospitalization in patients undergoing CRT.MethodsStudies comparing LBBAP and BIVP for CRT in patients with HF with reduced left ventricular ejection fraction (LVEF) were included. The coprimary outcomes were all-cause mortality and HF-related hospitalization. Secondary outcomes included procedural and fluoroscopy time, change in QRS duration, and change in LVEF.ResultsThirteen studies (12 observational and 1 RCT, n= 3239; LBBAP= 1338 and BIVP= 1901) with a mean follow-up duration of 25.8 months were included. Compared to BIVP, LBBAP was associated with a significant absolute risk reduction of 3.2% in all …

A summary of fatal injury surveillance methods in Australian agriculture and their impact on safety policies and practices

Agriculture is one of the most important and also hazardous industries in Australia. Having a sound knowledge and understanding of the circumstance of injury events is critical to developing evidence-based intervention programs. This paper aims to provide a brief historical snapshot of the development of data systems underpinning the assessment of fatal farm injury in Australia and how it has impacted on safety policy and practice. The first Australian studies used coronial information to explore agricultural fatalities, these studies reviewed paper-based records (in-situ) and collected the information for analysis and reporting. This task was laborious and costly. When the National Coronial Information System (NCIS) was established in 2000, this allowed access to coronial records online. Information provided about the deceased includes demographics, contextual details on the nature of the fatality and autopsy …

Health effects associated with chewing tobacco: a Burden of Proof study

Chewing tobacco use poses serious health risks; yet it has not received as much attention as other tobacco-related products. This study synthesizes existing evidence regarding the health impacts of chewing tobacco while accounting for various sources of uncertainty. We conducted a systematic review and meta-analysis of chewing tobacco and seven health outcomes, drawing on 103 studies published from 1970 to 2023. We use a Burden of Proof meta-analysis to generate conservative risk estimates and find weak-to-moderate evidence that tobacco chewers have an increased risk of stroke, lip and oral cavity cancer, esophageal cancer, nasopharynx cancer, other pharynx cancer, and laryngeal cancer. We additionally find insufficient evidence of an association between chewing tobacco and ischemic heart disease. Our findings highlight a need for policy makers, researchers, and communities at risk to devote …

Mesoporous bimetallic S-doped nanoparticles prepared via hydrothermal method for enhanced photodegradation of 4-chlorophenol

Magnesium oxides (MgO) have gained shown significant promise for a variety of applications, which can be modified by ions doping. In this study, bimetallic Ag-doped S–MgO nanoparticles were prepared by hydrothermal method and used for photocatalytic degradation of 4-chlorophenl (4-CP). EDX suggested the presence of no impurities, which mainly contained Mg, Ag, and S elements, suggesting that S and Ag were incorporated into the lattice of MgO as a result of successful doping. Estimated bandgap of Ag-doped S–MgO nanoparticles was 3.7 eV, lower than MgO (7.8 eV), but useful to improve optical characteristics and photocatalytic efficiency to degrade 4-CP up to a maximum of 99.60 ± 0.50%. The synergetic parameter during photocatalysis of 4-CP was 6.91, confirming the degradation of 4-CP. Quenching experiments proved the presence of hydroxyl radicals (•OH) and singlet dioxygen (1O2) that were …

Cortisol as a Marker of Pain and Distress After Acute Musculoskeletal Trauma

Objectives:The best available predictors of chronic pain development broadly encompass baseline metrics of cognition (ie, beliefs/expectations) about the trauma and resulting symptoms. In the context of musculoskeletal trauma, we have previously shown the Traumatic Injuries Distress Scale (TIDS) capable of risk-stratifying cohorts for chronic pain development. Here, we explore whether the physiological marker cortisol shows meaningful associations with cognitions predictive of pain outcomes. Methods:Data for these cross-sectional analyses were drawn from an observational study that recruited 130 participants presenting to the hospital with pain related to a recent noncatastrophic Musculoskeletal trauma. Cortisol was measured from the participant’s hair, saliva, and blood. In addition to pain and distress questionnaires, metadata such as age, sex, body mass index (BMI), adverse childhood events, pretrauma …

BackgroundDetailed, comprehensive, and timely reporting on population health by underlying causes of disability and premature death is crucial to understanding and responding to complex patterns of disease and injury burden over time and across age groups, sexes, and locations. The availability of disease burden estimates can promote evidence-based interventions that enable public health researchers, policy makers, and other professionals to implement strategies that can mitigate diseases. It can also facilitate more rigorous monitoring of progress towards national and international health targets, such as the Sustainable Development Goals. For three decades, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) has filled that need. BackgroundRegular, detailed reporting on population health by underlying cause of death is fundamental for public health decision making. Cause-specific estimates of mortality and the subsequent effects on life expectancy worldwide are valuable metrics to gauge progress in reducing mortality rates. These estimates are particularly important following large-scale mortality spikes, such as the COVID-19 pandemic. When systematically analysed, mortality rates and life expectancy allow comparisons of the consequences of causes of death globally and over time, providing a nuanced understanding of the effect of these causes on global populations.MethodsThe Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 cause-of-death analysis estimated mortality and years of life lost (YLLs) from 288 causes of death by age-sex-location-year in 204 countries and territories and 811 subnational …

Burden and quality of care index of chronic kidney disease: Global Burden of Disease analysis for 1990–2019

Background Chronic kidney disease (CKD) imposes a heavy obscure burden on individuals and health systems. Besides its burden, the quality of care of CKD is less well investigated. In this study, we aimed to explore the global, regional and national trends of CKD burden and quality of care. Methods The Global Burden of Disease Study 2019 data were used. Trends of incidence, prevalence, deaths and disability-adjusted life years were studied for the 1990–2019 period in the global aspect. By generating four secondary indices to assess different aspects of quality of care the quality of care index (QCI) was developed to explore the care provided for CKD. Inequities and disparities between various geographic, socio-demographic and age stratifications, and sex were studied using the QCI values. Results In 2019, there were 18 986 903 (95% uncertainty …

A first in disease phase 2, randomised, controlled trial of Granulocyte-Macrophage Colony-Stimulating factor neutralisation for Axial Spondyloarthritis (NAMASTE study)

Background: Granulocyte-macrophage colony-stimulating factor (GM CSF) is a proinflammatory cytokine that is overproduced in several inflammatory and autoimmune diseases, including axial spondyloarthritis (axSpA). Namilumab is a human immunoglobulin G1 monoclonal anti GM-CSF antibody that potently neutralises human GM CSF ligand. We aimed to investigate whether GM-CSF inhibition is a safe and effective treatment in patients with active axSpA.Methods: In this randomised, double-blind, placebo controlled phase 2 trial (NCT03622658), adults with active axSpA despite treatment with at least two different non-steroidal anti-inflammatory drugs (NSAIDs), and/or previous inadequate response or intolerance to anti tumour necrosis factor (TNF) therapy, were randomised via an Interactive Web Response System (IWRS) in a Bayesian 6: 1 (active drug: placebo) ratio to receive either four subcutaneous injections of namilumab 150 mg or placebo over 10 weeks on Weeks 0, 2, 6 and 10 at nine hospitals in the United Kingdom. The primary endpoint was the Assessment in SpondyloArthritis international Society (ASAS) 20 response at Week 12.Findings: From September 06, 2018, to July 25, 2019, 42 patients were randomized to namilumab (n= 36, 86%) or placebo (n= 6, 14%). Mean age was 39.5 years (SD 13· 3), 17 (40%) were women and 7 (17%) had prior anti-TNF therapy. The study failed to meet its primary outcome measure. At Week 12, 14/36 (39%) participants in the namilumab group achieved ASAS20 clinical response versus 3/6 (50%) in the placebo group. Hip-focused physical therapy versus spine-focused physical therapy for older adults with chronic low back pain at risk for mobility decline (MASH): a multicentre, single-masked …

BackgroundPreviously, we identified a population of older adults with chronic low back pain, hip pain, and hip muscle weakness who had worse 12-month low back pain and functional outcomes than age-matched adults with only low back pain, indicating an increased risk for future mobility decline. We sought to determine whether tailored, hip-focused physical therapy reduced pain and functional limitations in this high-risk population compared with non-tailored, spine-focused physical therapy.MethodsWe did a multicentre, single-masked, randomised controlled trial at three research-based sites in the USA. We recruited older adults (aged 60–85 years) with hip pain and weakness who reported moderate low back pain intensity at least half the days in the last 6 months. Patients were randomly assigned to hip-focused physical therapy or spine-focused physical therapy using permuted blocks with random block size …

Global, regional, and national burden of neck pain, 1990–2020, and projections to 2050: a systematic analysis of the Global Burden of Disease Study 2021

BackgroundNeck pain is a highly prevalent condition that leads to considerable pain, disability, and economic cost. We present the most current estimates of neck pain prevalence and years lived with disability (YLDs) from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) by age, sex, and location, with forecasted prevalence to 2050.MethodsSystematic reviews identified population-representative surveys used to estimate the prevalence of and YLDs from neck pain in 204 countries and territories, spanning from 1990 to 2020, with additional data from opportunistic review. Medical claims data from Taiwan (province of China) were also included. Input data were pooled using DisMod-MR 2.1, a Bayesian meta-regression tool. Prevalence was forecast to 2050 using a mixed-effects model using Socio-demographic Index as a predictor and multiplying by projected population estimates. We present …

The Florida Scoring System for stratifying children with suspected Sjögren's disease: a cross-sectional machine learning study

BackgroundChildhood Sjögren's disease is a rare, underdiagnosed, and poorly-understood condition. By integrating machine learning models on a paediatric cohort in the USA, we aimed to develop a novel system (the Florida Scoring System) for stratifying symptomatic paediatric patients with suspected Sjögren's disease.MethodsThis cross-sectional study was done in symptomatic patients who visited the Department of Pediatric Rheumatology at the University of Florida, FL, USA. Eligible patients were younger than 18 years or had symptom onset before 18 years of age. Patients with confirmed diagnosis of another autoimmune condition or infection with a clear aetiological microorganism were excluded. Eligible patients underwent comprehensive examinations to rule out or diagnose childhood Sjögren's disease. We used latent class analysis with clinical and laboratory variables to detect heterogeneous patient …

Gout and incidence of 12 cardiovascular diseases: a case–control study including 152 663 individuals with gout and 709 981 matched controls

BackgroundGout, a common crystal arthropathy, is associated with increased risk of cardiovascular disease. We aimed to identify how this risk varies by individual cardiovascular disease across a broad spectrum of conditions.MethodsIn this matched case–control study, we used linked primary and secondary electronic health records from the UK Clinical Practice Research Datalink to assemble a cohort of individuals with a first-time diagnosis of gout between Jan 1, 2000 and Dec 31, 2017, who were aged 80 years or younger at diagnosis, and free of cardiovascular diseases up to 12 months after diagnosis. The control cohort comprised up to five control individuals per patient with gout, matched on age, sex, socioeconomic status, geographical region, and calendar time, randomly selected among individuals free of gout at any time before and during the study period. The cohorts were followed up until June 30 …

Effects of tapering conventional synthetic disease-modifying antirheumatic drugs to drug-free remission versus stable treatment in rheumatoid arthritis (ARCTIC REWIND): 3-year …

BackgroundTapering of disease-modifying antirheumatic drugs (DMARDs) to drug-free remission is an attractive treatment goal for patients with rheumatoid arthritis, although long-term effects of tapering and withdrawal remain unclear. We compared 3-year risks of flare between three conventional synthetic DMARD treatment strategies in patients with rheumatoid arthritis in sustained remission.MethodsIn this open-label, randomised controlled, non-inferiority trial, we enrolled patients aged 18–80 years with rheumatoid arthritis who had been in sustained remission for at least 1 year on stable conventional synthetic DMARD therapy. Patients from ten hospitals in Norway were randomly assigned (2:1:1) with centre stratification to receive stable conventional synthetic DMARDs, half-dose conventional synthetic DMARDs, or half-dose conventional synthetic DMARDs for 1 year followed by withdrawal of all conventional …
